The ESRD population is at high risk for drug-related problems and supports the role of a clinical pharmacists in this healthcare setting. Consultant pharmacists who care for these patients are required to manage an extensive list of medications.

  • Provide ongoing Medication Regimen Reviews (MRR).
  • Perform unit inspections.
  • Monitor and assist with all medication management policies, procedures, and survey plan.
  • Collaborate with staff on special projects and in-servicing.

The New Jersey Department of Health and Senior Services regulations for Chronic Dialysis Services require facilities to retain a consultant pharmacist who has no affiliation with the pharmacy provider of facility medications.
